Emergency services are responding to a three-car crash on State Highway 1 north of Puhoi that has left one person fighting for their life.
State Highway 1 is closed near Schollum Access Rd following the serious crash.
Police were called to the incident just before 5pm.
"One person was trapped in a vehicle and has been cut free. A helicopter is at the scene now and the person will be transported to hospital, they are in a critical condition," a police spokeswoman said.
Other occupants are all out of the vehicles and are being tended to by ambulance staff at the scene.
The road is closed in both directions while the Serious Crash Unit investigates.
Diversions are in place at the intersection of SH1 and Moir Hill Rd, and SH1 and Woodcocks Rd.
Police advise motorists to delay travel where possible and expect delays.
A St John Ambulance spokeswoman confirmed paramedics were at the scene, assessing one patient in a critical condition and one in a moderate condition.
Three ambulances and a helicopter were in attendance.
Local Roy Hyslop said he could see the helicopter parked in the metal pit beside the road, and that traffic was still moving.
He felt the speed limit should be reduced after observing a number of accidents along Windy Ridge and Schedewy's Hill.
