LONG TERM CONDITIONS LEAD

Eastern Bay Primary Health Alliance (EBPHA) has a vacancy for a Long Term Conditions Lead. The Long Term Conditions Lead is responsible for the facilitation and coordination of the Cardiovascular Disease (CVD)/Diabetes Self-Management programme.
The key responsibilities are to:
? Achieve health outcome targets as described in the strategic plan;
? Raise the profile of self-management across the region with both key stakeholders and the community;
? Promote self-management of Heart Health and Living Well with Diabetes to the local community;
? Support General Practice in the treatment of patients with Long Term Conditions;
? Facilitate self-management sessions across the region;
? Coordinate input of guest speakers to the programme.
Essential qualifications for this role are;
? A New Zealand Registered Nurse;
? At least five years' experience in leading work teams with a track record of demonstrable success;
? Genuine understanding of nursing matters, in particular Diabetes and CVD programs, public health and health promotion;
? Experience in delivering health education programmes;
? Experience in developing and implementing strategic and management plans;
? Experience in successfully managing operations within a budget.
