"The truck driver has significant leg injuries and has been taken to the Royal Brisbane Hospital," the department said in a statement.
"Paramedics assessed a further 15 patients on scene, 14 who were on the train at the time of the incident and the train driver. No one assessed on scene is reported to be injured."
Witness Chris Harmon told ABC radio the truck was sitting across the tracks when it was struck by a northbound train.
"It appears a truck got held up in traffic across the train crossing. The northbound train collided with the truck and shunted the truck into the southbound train," he told ABC radio.
He said he was on the southbound train, and he and his fellow passengers were quickly herded off, with rail authorities saying power lines had been damaged.
"You have a semi-trailer wedged between two trains," he said, adding the accident would take some time to clear.
