The woman ended up in the 4WD vehicle, which landed on its side, in a stream off the Dairy Flat Highway, north of Albany on Auckland's North Shore.
It appeared the woman had simply veered off the road, before striking a cabbage tree and rolling about 10 metres down the bank into the creek.
Fire Service spokesman Paul Radden said two fire crews from Albany and Takapuna were dispatched to rescue the woman, believed to be about 70-years-old, from the crash.
The woman was recovered from the vehicle about 30 minutes after the crash, about 11.30am.
"We found the vehicle in the stream and one person was trapped inside the vehicle. The person was extricated from the car and taken up the steep bank, then placed in the care of ambulance staff."
An ambulance spokeswoman said the woman was transported to Auckland Hospital by ambulance, with "serious injuries".
"We did send the helicopter, but she ended up going by road ambulance," she said.
