Denzel Washington plays John Q. Archibald, a Chicago factory worker whose son collapses during a baseball game. John Q and his wife, Denise (Kimberly Elise), race him to hospital, where a cardiologist (James Woods) explains there are two options: a heart transplant or optimising Mike's "quality of
life".
Hospital administrator Rebecca Payne (Anne Heche) knows the family has no money and argues for the second choice. John Q thinks he is covered by medical insurance but finds his worker's contributions won't cover the $250,000 operation.
John Q pulls a gun and takes hostages, demanding that his son be put at the top of the transplant list. Cops surround the hospital and send in negotiator Grimes (Robert Duvall), while police chief Monroe (Ray Liotta) grandstands for TV cameras. Diagnosis: turn off the life support.
