Traffic was backed up for more than an hour on State Highway 3 near Ratana after a collision believed to involve three cars.
Emergency services were called to the scene just before 12pm.
A St John spokeswoman said three patients with moderate injuries were taken by ambulance to Whanganui Hospital.
