A mental health patient who allegedly attacked a woman with a sword has prompted a Ministry of Health investigation into a clinic.
The South Auckland man was under the care of the Auckland-based Mason Clinic when the ceremonial sword attack allegedly happened on November 4.
Singh was accused of attacking the woman at her home in Hamilton, leaving her with severe injuries.
The Ministry of Health today said an external inquiry would focus on management and treatment of the man at the community-based clinic.
The ministry said forensic psychiatry consultant Dr Nick Judson and mental health district inspector David Niven would carry out the inquiry.
