A teenager died yesterday after falling five floors at the Imax Force Entertainment Centre in Queen St, central Auckland.
The youth, aged 16, was believed to have been sitting on a ledge on the centre's cinema floor when the accident happened.
Occupational Safety and Health officer Fred Ray said it was not clear what the boy was doing or why he fell. He estimated the fall to be 30m.
The teenager was taken to Auckland Hospital in a critical condition and died later. Mr Ray said he understood the boy suffered severe head injuries.
Imax spokeswoman Vanessa Regan said no details would be made available until a report from OSH was completed. She said witnesses had given "100 different versions."
Asked if management would consider making safety changes at the Imax centre, Ms Regan said that issue would have to wait until the OSH report was released.
The building came under scrutiny when it opened because of its open-plan design.
The escalators reach through the centre of the building with just a handrail for safety.
